RESPOND TO CLAIMANT WAGE CREDIT POST AUDIT NOTICE UCO-2

In order to verify claim information employers should file a response to the Claimant Wage Credit Post Audit - UCO-2. Eligible separated employees have the right to file a claim for reemployment assistance. Furnishing accurate information helps prevent reemployment assistance fraud and protects employers’ tax rates. Please provide wage information carefully because it may be used in legal action.

Please be advised that the Department of Economic Opportunity’s Reemployment Assistance Program only accepts Department-approved forms for processing claims information.

Please use the forms provided by the Department. In accordance with section 443.151(3)(a), F.S., employer accounts will not be relieved of benefit charges if a response to a notice of claim or request for information is submitted on anything other than a Department-approved form. You can submit the form via fax to the fax number provided on the form or submit the form online using CONNECT by logging in to your employer inbox at: https://employers
.connect.myflorida.com.
