Step B - Provider Acknowledgement

- Complete contact information for the person who is submitting the Provider Acknowledgement. You will be prompted to answer seven questions about your VPK program.
- Click in the box to acknowledge the Low Performing Provider statement. You will not be able to continue to the Improvement Plan if this is not checked. You will receive an error message that looks like this [Click here for an example].

- List the total number of your qualified VPK instructors. This does not include other instructors that may be involved in your program.
- Indicate the VPK instructors' level of education as they pertain to their qualifications to teach VPK.
- Indicate how many of the VPK instructors have degrees in early childhood.
- If your totals do not equal the total number of qualified VPK instructors, you will receive an error message and will not be allowed to go any further.

- Indicate the number of VPK classrooms in which the primary language of instruction is English, Haitian-Creole, Spanish, or other.

- Indicate the number of your VPK instructors who have completed the VPK Education Standards Training
- Indicate the number of your VPK instructors who have completed the Five-Hour DOe Emergent Literacy Training

- For each program year, check the box next to the curricula that was implemented in your VPK program. Check all that apply. If the curricula that you used in your program is not listed, fill in the "Other" blank with the name of the curriculum.

- Answer "Yes" or "No" for each program year, if the children in your VPK program have been administered developmental assessments.
- A developmental assessment can be both an informal or a structured evaluation of a child's development — physical, language, intellectual, social, and emotional — by a developmental assessment specialist, or a team of professionals that can include a pediatrician, language specialist, audiologist, occupational therapist, child psychologist, and child psychiatrist, among others.
- Check all that apply. If the assessment that you used in your program is not listed, fill in the "Other" blank with the name of the assessment.

- Answer "Yes" or "No" for each program year, if the children in your VPK program have been administered developmental screenings.

Developmental screening is a short, quick procedure designed to identify children who should receive more intensive assessment or diagnosis, for potential developmental delays. Developmental screening can allow for earlier detection of delays and improve child health and well-being for identified children as well as inform teacher practices.

- Answer "Yes" or "No" if your VPK classrooms have had an environmental assessment within the last year.

An environemental assessment is the process of measuring, collecting and interpreting information about an early childhood program. An environmental assessment considers elements such as space arrangement, furnishings, health, safety, adult-child interaction, materials, activities, supervision, and program structure.
