Eligible separated employees have the right to file a claim for reemployment assistance. In order to verify claim information employers should respond to a Claimant Wage Credit Post Audit Form (UCO-2) from the Department of Economic Opportunity.

Furnishing accurate information helps prevent reemployment assistance fraud and protects employers’ tax rates. Please provide wage information carefully because it may be used in legal action.
